Project: Shape Up San Francisco
This grant is intended for chronic disease prevention efforts. The goal of this initiative is to reduce risk factors, prevent or delay chronic disease, promote wellness in children and adults, and provide positive, sustainable health change in communities. This funding will be used to address the leading preventable causes of death and disability, namely obesity and tobacco use, by expanding the use of evidence-based strategies and programs, mobilizing local resources at the community-level, and strengthening the capacity of states. This grant program is administered by the U.S. Department of Health and Human Services.
Why did San Francisco apply for this grant?
San Francisco would use this grant to implement Shape Up San Francisco (SUSF), a comprehensive city-wide program that focuses on interventions that include building on the City’s General Plan and its related policies, using media to promote healthy eating and active living as the norm, and to create the settings and opportunities for San Francisco residents to eat well and move more. In addition to initiatives citywide in nature, SUSF proposes numerous interventions that will specifically focus on reducing chronic disease health disparities in six specific neighborhoods: Bayview Hunters Point, the Tenderloin, South of Market, Western Addition, Visitacion Valley, and the Mission.
